Three ways you can win!!!!

1. Each UPHA member who sponsors at least two (2) new members to join UPHA between 1/1/14 and 5/15/14 will be entered in a drawing, and the winner will receive a

$125.00 gift certificate to a UPHA/Merchant Partner* (winner’s choice) or $100.00 to any Hillsborough St. merchant.**

2. Each new member joining between 1/1/14 and 5/15/14 will be entered in a second drawing, and the winner will receive a

$75.00 gift certificate to a UPHA/Merchant Partner* (winner’s choice) or $50.00 to any Hillsborough St. merchant.**

3. The UPHA member to sponsor the most new members will win

$100.00 in cash. In case of a tie, a drawing will be held to determine the winner.

How to enter the contest:

1. Obtain and print a UPHA Membership Form located at the UPHA website (Membership Form) or contact a UPHA Board member for a form. A list of the current UPHA Board Members can be found at the website https://myupha.com/about/board-of-directors/

2. Document your sponsorship on the form:

“Sponsor: Your Name” to receive credit for each new member you sponsor.

3. Have the University Park resident complete the form and mail it to UPHA (P.O. box 10343, Raleigh, NC, 27607) with a check for $100.00.***

4. This contest begins 1/1/14 and ends in 5/15/14, after which a public drawing will be scheduled; time, date, and location to be announced later.

5. All completed membership forms and full payment must be received in the UPHA P.O. box by 5/15/14 to be entered in the drawing.
